You won't need anything except household items and some inexpensive and easily obtainable equipment to do this program (your local Mart or discount store).
However, having said that, if you have a stationary bike, that can be used in the program. You can also buy an adaptation for a bicycle to make it stationary and it's very inexpensive. I have been searching the internet and when I find it, I will link it from here. There is a link at the bottom for a small pedal exerciser that will work. So some kind of bike is helpful.
Your legs. Of course treadmills can be used instead of walking outside. If you use a treadmill that does not have a distance counter, you need to pick one up that you can wear. Measure your outside distance in your car if possible.
You will need a Stairs (or Stairmaster) or a sturdy step-stool (the higher, the harder) and a soft place to lay and do exercises. I actually do them on the bed or couch while I watch the news. An exercise pad is great, but don't buy the cheap ones - they don't really provide enough padding.
You will need two dumbbells and I like the ones where you can switch out the weights as you get stronger, but they are cheap and you can buy more when you need to if you prefer the solid ones.
I like to use leg weights, especially for martial arts training. They can make you lightning fast and will certainly build up strength. Buy the ones that have velcro that you can tighten to fit you. Do not overestimate - start with a small weight at first and build up.
You may need a chair to hold on to as you do some of these exercises. I find a folding chair works best for me. It's just sturdy enough and easy to put away. This is mostly for Level 1 and Senior Level.
A jump rope is only needed if you prefer that over walking or biking.
An exercise ball is great for Level 2 and Level 3 to do some of your weight training on.
The one other piece of equipment that I may suggest is a stretch cord or something that you can pull on with two handles for resistance training. Some muscles are hard to work without that pulling motion.
Oh - throw your scales out the window. Like we need more pressure :)
http://www.rehabmart.com/exercise.asp has the Pedlar Leg/Arm Exerciser (bike replacement), Thera-Band Therapy Balls, and Theraband® Resistive Exercise Band. I am not personally recommending these, just trying to make things easier to find. It seems they are reasonably priced.

has the
Stretch
Cords - TKO Exercise Stretch Cords for $11.99. I
like these.
If you have used equipment that you would like to sell, please list them on our site so that others can find them easily.
